Depending on his or her preference, your doctor may order a saliva, blood or urine test to measure your cortisol. These days it is generally accepted that saliva cortisol testing is the most accurate, as it gives a better estimate of the cortisol levels within your cells, where the hormone reactions are actually taking place. Remember that it is important to be well-hydrated before you do your saliva test – dehydration can skew the results.

To assess adrenal function, we conduct a one-hour ACTH stimulation test, which is very safe and reliable. We also look for thyroid issues and diabetes. After checking for a variety of endocrine disorders using testing and symptom assessment, we start working with a multidisciplinary team of doctors to figure out the root cause of the patient’s problem.
If you have tiredness, brain fog, lack of motivation, among other symptoms, you should first have a thorough evaluation with a medical doctor. Anemia, sleep apnea, autoimmune diseases, infections, other hormonal impairments, mental illnesses, heart and lung problems, and kidney and liver diseases are just some among many medical conditions that could cause similar symptoms. If the workup from your medical professional turns out normal and you believe you might have adrenal fatigue, I would recommend you consider a fundamental question: Why would your adrenals be drained? Take a better look at what types of stress might be affecting you. For many, the hectic pace of modern life is to blame.
Low adrenal hormone levels may also be linked to low blood sugar. Normally, cortisol works to maintain blood sugar levels throughout the day, but low cortisol levels may not be sufficient to sustain blood glucose. When blood sugar levels drop, sleep is disrupted, which can cause you to wake earlier than normal – for example, between 1-3am. Eating a small, healthy snack before bedtime may help to combat this. Choose foods that are low in sugar and high in protein and/or healthy fats in order to help stabilize blood sugar without spiking it.
Dr. Smith will likely order blood work, but in some cases salivary cortisol levels taken at four different times over the course of the day (a kit you take home) is a better option for a more detailed survey of adrenal function. Usually Dr. Smith will also assess some other adrenal hormones in her lab work such as DHEA. Both blood and salivary tests are equally sensitive when testing cortisol levels.
Conventional medicine will detect only the extremes of these conditions, when damage to the adrenals has already occurred (Cushing’s disease and Addison’s disease). Within those extremes, you can feel miserable and still be told your cortisol levels are normal. But by responding to early-stage symptoms of adrenal fatigue, we can reverse the developing dysfunction.

With some simple questions I might make a few basic lifestyle recommendations, talk about the evidence supporting supplements and vitamins, and suggest physician follow-up if symptoms persist. Fatigue and stress may be part of life, but they’re also symptoms of serious medical conditions. But they can be hard to treat because they’re non-specific and may not be easily distinguishable from the fatigue of, well, life.
But this sudden onset of foggy brain become chronic for the last 3 years. Visited multiple doctors. Took Brain MRI, Multiple blood test, Vitamin B12 went down and had shots to recover, no kidney issues. All the blood test came back fine. Tried Yoga and meditation but no use. Insomnia & also sometimes sleeping more. Got many food allergies like Gluten and Dairy products. Tried sports & other exercises but no recovery. When i wake up in the morning i feel like i did not sleep and have the sensation of lightheadedness.
Wondering if any of you have this happening…my symptoms are worse each month around the same day the 20th. Starts out feeling flu like massive headache, body aches slight fever, nausea. And in 2-3 days it starts to dissipate and then moves to my lower back and nerve like radiating pain in my lower back. Usually lasting 2-3 days, this month it came earlier and lasted longer 6 nights to be exact. Happens like this each month. Today, the Endocrinologist told me it’s not hormones/menopausal. What the heck!!
I am 29 years old and I have had Gastric Bypass Surgery. I have lost 150 lbs since 2013. I have a lot of they symptoms that are described. I have a hard time getting out of bed, mild depression, brain fog, I get lightheaded when I stand up daily. I do drink a lot of caffeine about 2 energy drinks a day for the past 2 1/2 years. I fee like it is the only thing that can give me the energy to function. lately my blood sugar has been all over the place and I feel like I constantly have to snack in order to maintain feeling normal. I am on a Thyroid medication and my Thyroid is normal with that. My periods have been really off lately, I went to my Gyno who just put me on birth control. I want to see about going to an internal medicine doctor who can help me. because I don’t know what to do to feel better.

Spending more time outside in the sun also helps boost levels of vitamin D, because your body manufactures this important vitamin/hormone when it senses sun on your skin. Vitamin D, is responsible for regulating over 200 genetic pathways, so make sure your levels are high enough. I recommend an optimal range of around 60 to 80 ng/ml. Ask your doctor about a simple blood test to help you keep track.
Conventional blood tests, taken at whatever time your doctor has scheduled your appointment, might indicate that your adrenals are normal. However, a better diagnostic approach will test your levels at different times of the day, which is much more likely to reveal an out-of-whack pattern of cortisol or DHEA secretion. I was diagnosed with severe adrenal fatigue and have been under a naturopathic dr’s treatment for months and have improved somewhat. in the beginning when I crashed, my blood pressure was very low. Now that I am on the road to healing (and its such a long road), the bottom number of my blood pressure continues to rise until now it is almost always in the upper ’90’s. The top number has remained great. Have you heard of this before? I’m wondering what is causing my bottom number to rise and what I can do about it?
My life started to get better when I read this book. Every single improvement in my health over the past six months, and there have been many, have come from putting the advice in this book into action. I can not recommend it highly enough to people who want to do more, but just simply can not summon the energy and are tires of not knowing why they are so tired.
While adrenal fatigue may not exist, the same can’t be said for the treatments. When you’re treating a fake disease, anything goes. Everything from homeopathy to herbal remedies to hydrotherapy, to traditional Chinese medicine and vitamin supplements are advocated for treatment. The endpoints of treatment are as nonspecific as the criteria for diagnosis. Wilson, conveniently, has his own supplement programs, and there’s no shortage of solutions out there.
Just to add to my comment above. After taking Siberian Ginseng for about 8 weeks, I had a break for 2 weeks and started back on it again. Yes, taking it in the afternoon has left me with insomnia for a couple of nights and also an upset/colicky stomach so this is quite powerful stuff. I’m sure it’s brought on my next period a couple of days early too.
Exercise gently at least 5 days per week, preferably morning and not late in the day.  Try swimming, gentle bike rides, walking, yoga, stretching, or any restorative activity that incorporates breathing.  Avoid high adrenaline activities, like kayaking, competitive sports, running, cross fit or high intensity training until you are well on your way to healing.
No ground is left uncovered in this book, as Dr. Wilson brings all of his experience and knowledge to bear in a Herculean effort to bring some semblance of ordered understanding to what had previously been a chaotic stew of confusing and sometimes contradictory assumptions. Everything is methodically explained, referenced, and indexed so that curious readers can do further research on their own to confirm the author’s premise and findings. He even includes a Glycemic Index table, a glossary of terms, and a host of illustrations scattered throughout the book’s various pages.
Stress is not a bad thing. We all have stress in many forms but the key is to be able to handle it and to balance your life in a way so that you do not stay in the fight/flight mode all the time. When that happens, you’ll have insomnia, belly issues/digestion issues, exhaustion, hormonal imbalances, weight gain, thyroid issues and many other symptoms. Your immune system will also be very suppressed so you’re susceptible to getting infections and viruses/colds or even catching something like Lyme disease. Sound familiar? I’m pretty much telling you what happened to me. My nervous system was knocked out of balance so much that my body couldn’t perform it’s regular duties and everything started to shut down because of emotional, physical and mental stressors that I was put under when I was younger, which is why I was a prime case for Adrenal Fatigue Treatment.

Sugar and sweeteners: Includes avoiding high-fructose corn syrup and artificial sweeteners as well. Avoid sugary foods, cereals, candy, sweets, etc. Be aware that sugar is an additive in many breads, condiments and dressings. Try to avoid as much extra sugar as possible. Seek the benefits of raw honey or stevia as an alternative, and always moderate your use of sweeteners of any kind.
A saliva cortisol test is taken at home using a simple kit. Basically, you spit into a vial several times a day, then send the samples in for analysis. The lab will review how your cortisol levels ebb and flow. I’ll explain more in future posts, but for now what you need to know is that an abnormal pattern suggests HPA axis dysfunction, which suggests adrenal dysfunction. (HPA stands for hypothalamic-pituitary-adrenal.)

Well that explains it Dr. Northrup! After a difficult winter I was on my way to rejuvination and ran into a road hazard without warning. I have been freaking out ever since. Very concerned someone else will hit it and not be so lucky. I have withdrawn from my usual uplifting activities and feel as if I will emerg reborn. Living by a motto of “Eat right, get plenty of rest and exercise” has worked for decades, menopause is a trip! I will be preparing delicious meals to savor and add Licorice and Ginseng to replace caffine. Practice having more fun while “Celebrating peace and love always”. I was shaken hard enough in the accident that my pinky and ring fingers are numb, it was spreding to my palm and wrist but numbness has receeded to fingers. I was restless and having nightmares too. My hump on the back of my neck flattened. I am doing PT to keep it away. Sleeping with arms straight and down seems to help too. Driving can be stressful as it requires we be more alert. I had taken a side road to get away from a driver who was swerving and driving at inconsistant speeds and a tailgater who had been behind me for sometime on a dangerous two lane road know for head ons and off road accidents. Hummm, getting back to focusing on love and having more fun. . .Now!!!

I dealt with adrenal fatigue before I developed Bulletproof. It was hard to figure out what was wrong because a lot of doctors didn’t (and still don’t) recognize adrenal fatigue as real. One big reason is that when your stress response is messed up, it affects so many parts of your biology that it’s hard to identify as adrenal fatigue, and even harder to study.
