A prospective, observational clinical trial examined the endocrine effects of Tribulus terrestris 750 mg per day, over five days in eight healthy women (aged 28–45). A significant increase in mean serum FSH concentration from 11 mIU/ml before treatment to 17.75 mIU/ml following treatment (P < 0.001) was demonstrated. Pre-treatment FSH levels returned following cessation of treatment (Table 1). Another clinical study evaluated the equivalence of Tribulus terrestris (Tribestan®) and pharmaceuticals for ovulation induction in women with oligo/anovular infertility (n = 148), [60]. During the three month follow up, ovulation rates were highest with epimestrol (74%), followed by Tribulus terrestris (60%), clomiphene (47%) and cyclofenil (24%). However, the evidence for Tribulus terrestris should be interpreted with caution due to risks for bias in clinical studies. One study was uncontrolled with a small number of healthy participants [56], the second study did not report baseline characteristics, methods for allocation to treatment groups and data were not statistically analysed [60] (Table 1).

Due to the high cost of the procedure, the need for hospitalization, general anesthesia and higher complications risks, ovarian drilling presents low cost effectiveness compared with gonadotropin plus timed intercourse. Moreover, the lack of standardization of the surgical technique and the absence of studies that have evaluated the repercussions of long-term of ovarian drilling demonstrate that this procedure should not be routinely performed but should only be considered as second line of therapy in women with PCOS who will be undergoing laparoscopy for another reason (adnexal mass or pelvic pain, for example). Additionally, ovarian drilling could be an alternative before the assisted reproduction treatment (ART) in individuals without financial conditions for the realization of ART and those who are resistant to CC.
Getting three hours of exercise a week is enough to improve insulin sensitivity in women with PCOS, especially if you have been inactive up until now. Exercise can help you lose more weight, but more importantly, it can also help you lose the fat around your abdomen – that’s what contributes directly to insulin resistance, which fuels PCOS symptoms – even if you don’t lose any weight. That’s what happened in a 2017 small Monash University study.16  Women in this study added interval training (ie, short bursts of high-intensity exercise followed by regular or moderate intensity intervals) for part of their workout.16
If a woman's weight is excessive, the physician should be aggressive in championing a weight-loss program. Medications effective for weight loss (in addition to lifestyle modifications) that have been specifically studied in women with PCOS include metformin, acarbose, sibutramine, and orlistat (Xenical). Metformin is probably the first-line medication for obesity or weight reduction in patients with PCOS. Metformin results in a decrease in body mass index (BMI) of 1 to 2 kg per m2 or weight loss up to 6 lb, 10 oz to 8 lb, 13 oz (3 to 4 kg)10,46,49; acarbose results in an approximate 3 kg per m2 decrease in BMI24; sibutramine results in a decrease in BMI of 5.8 kg per m2 and weight loss of 31 lb, 11 oz (14.4 kg)26; and orlistat results in weight loss of approximately 11 lb (5 kg).50 However, a recent systematic review suggested that metformin is not effective for lowering BMI in patients with PCOS.1

Clomiphene is an ovulation induction agent that has been used and studied in patients with and without PCOS.6–8,15,35,36 Studies have found that letrozole (Femara) regulates ovulation and improves pregnancy rates in women with PCOS6,37,38; however, this use is controversial because the drug is FDA pregnancy category D. It is embryotoxic and fetotoxic in animal studies, and there are no studies in pregnant women.

Cimicifuga racemosa was found to lower LH in two laboratory studies both examining cell cultures from ovariectomised rats [45, 52] (Table 1). The mechanism occurred through competitive inhibition of oestrogen following the selective binding of oestrogen receptors (ERα) on the hypothalamus and pituitary [52]. An earlier study found contrary results for reduction of LH, however this study investigated an isolated flavonoid and suggested that other constituents may be active [37].


Given the conditions associated with PCOS, the Endocrine Society, the Androgen Excess and PCOS Society, and the American College of Obstetricians and Gynecologists recommend that clinicians evaluate patients' blood pressure at every visit and lipid levels at the time of diagnosis, and screen for type 2 diabetes with a two-hour oral glucose tolerance test regardless of a patient's body mass index. Patients should have repeat diabetes screening every three to five years, or more often if other indications for screening are present.19–21 The Endocrine Society further recommends depression screening, as well as screening for symptoms of obstructive sleep apnea in overweight and obese patients with PCOS.19 However, routine screening for nonalcoholic fatty liver disease or endometrial cancer (using ultrasonography) is not recommended.19

Diagnosis can generally be accomplished with a careful history, physical examination, and basic laboratory testing, without the need for ultrasonography or other imaging. Hyperandrogenism can be diagnosed clinically by the presence of excessive acne, androgenic alopecia, or hirsutism (terminal hair in a male-pattern distribution); or chemically, by elevated serum levels of total, bioavailable, or free testosterone or dehydroepiandrosterone sulfate.23 Measurement of androgen levels is helpful in the rare occasion that an androgen-secreting tumor is suspected (e.g., when a patient has marked virilization or rapid onset of symptoms associated with PCOS).
Reproductive dysfunction in PCOS women may also be a manifestation of IR. Menstrual cycle irregularity has been correlated with HOMA-IR[37]. Molecular defects in insulin action may be responsible for reproductive difficulties in PCOS women. Although endometrial tissue appears morphologically similar in PCOS to controls and may have similar insulin receptor prevalence, insulin receptor action at the local endometrial level is impaired and may be reflected in lower pregnancy implantation rates[38]. HOMA-IR has been correlated with follicle count in PCOS during in vitro fertilization[39]. Follicular insulin levels correlate with pregnancy outcome after IVF[40]. These are areas of unresolved understanding with regard to PCOS. Proposed mechanisms for insulin reproductive abnormalities include abnormalities of ovarian steroidogenesis, excessive LH secretion and abnormalities in glucose uptake[41]. PCOS women have been found to have post-receptor insulin abnormalities as well as reduced peripheral insulin receptor binding[42].

OGTT with 75-g glucose and hourly glucose and insulin measurements has been compared to clamp techniques. Insulin sensitivity calculated by mathematical transformation of measurements has shown good correlation with glucose disposal using clamp techniques[48]. Although the OGTT is easy to perform, these calculations are more complex and make this particular calculation less desirable for clinical use. However these data show that 1 and 2 h levels are often needed to diagnose IR and stress the potential for false negative results with fasting measurements alone. In patients undergoing clamp and OGTT no correlation was observed between fasting glucose/insulin ratios and IR on the clamp[48].
Polycystic ovary syndrome (PCOS) is not a simple pathophysiologic process for which one treatment addresses all manifestations. It is a condition that occurs in approximately 5 to 10 percent of women of childbearing age.1 It can affect women in many different ways; therefore, physicians must individualize treatment goals and target treatment to specific manifestations. Comorbidities (e.g., cardiovascular risk factors, endocrinologic disease) and the patient's desire for pregnancy must be considered when choosing a treatment regimen.
Fasting methods to measure IR have been advocated for many years as an adjunct to DM screening. Elevated fasting insulin levels greater than 20 μU/mL may alone indicate IR. Fasting glucose/insulin ratio (G/I) has also gained some clinical traction. A ratio < 4.5 has in general been shown to be > 90% sensitive in some populations[45] but has never been validated with clamp studies[48]. Some ethnic variation in G/I cutoff ratios may exist[49]. There has been some suggestion that G/I < 7 in very young girls may predict IR[50,51].

Two important things to note are that the long term safety effects of use are not known (but the drug has been in use for decades already) and spironolactone IS NOT SAFE in pregnancy, so it is commonly used in conjunction with birth control pills. This use of birth control pills also protects against abnormal growth of the lining of the uterus. Some women cannot take birth control pills, in which case it is important to use other means of birth control if you are sexually active while taking spironolactone.
Side effects: Since licorice is one of the most widely used herbs there is a great deal of research into the possible side effects as well. It is definitely not recommended for long-term use because extended exposure may cause fluid retention, high blood pressure and potassium depletion. Methadone is said to not get people "high," but I beg to differ. It doesn't give any of the euphoria that heroin does, nor the rush, but it definitely has a body high to it. Lifestyle modification and weight reduction reduce insulin resistance and can significantly improve ovulation. Therefore, lifestyle modification is first-line therapy for women who are overweight.37 A calorie-restricted diet is recommended for all patients with PCOS who are overweight. Weight loss has been shown to have a positive effect on fertility and metabolic profile.19,30 The Endocrine Society recommends clomiphene or letrozole (Femara) for ovulation induction. Recent studies suggest that letrozole is associated with higher live-birth rates and ovulation rates compared with clomiphene in patients with PCOS.29 The impact of metformin on fertility is controversial; although it was once believed to improve infertility, a 2012 Cochrane review concluded that it does not.38

Because of the menstrual and hormonal irregularities, infertility is common in women with PCOS. Because of the lack of ovulation, progesterone secretion in women with PCOS is diminished, leading to long-term unopposed estrogen stimulation of the uterine lining. This situation can lead to abnormal periods, breakthrough bleeding, or prolonged uterine bleeding. Unopposed estrogen stimulation of the uterus is also a risk factor for the development of endometrial hyperplasia and cancer of the endometrium (uterine lining). However, medications can be given to induce regular periods and reduce the estrogenic stimulation of the endometrium (see below).

Pharmaceutical treatment for menstrual irregularity includes the oral contraceptive pill (OCP) and ovulation induction with clomiphene citrate (clomiphene) [20, 21] depending on fertility needs. Women with PCOS are however likely to exhibit contraindications for the OCP [3] and whilst induction of ovulation with clomiphene has demonstrated success, pregnancy rates remain inexplicably low [4]. Management for hyperandrogenism includes anti-androgens and hypoglycaemic pharmaceuticals such as metformin [24]. Metfomin has demonstrated effectiveness for improving insulin sensitivity and hyperandrogenism, however use of metformin is associated with the high incidence of adverse effects including nausea, vomiting and gastro-intestinal disturbances [5].

Diagnosis can generally be accomplished with a careful history, physical examination, and basic laboratory testing, without the need for ultrasonography or other imaging. Hyperandrogenism can be diagnosed clinically by the presence of excessive acne, androgenic alopecia, or hirsutism (terminal hair in a male-pattern distribution); or chemically, by elevated serum levels of total, bioavailable, or free testosterone or dehydroepiandrosterone sulfate.23 Measurement of androgen levels is helpful in the rare occasion that an androgen-secreting tumor is suspected (e.g., when a patient has marked virilization or rapid onset of symptoms associated with PCOS).

Oligomenorrhoea was defined as menstrual cycle length that extended beyond 35 days (day one being the first day of menses). Amenorrhoea was defined as no menstrual period for three to six months or more [19]. This review was focussed on hypothalamic, pituitary and ovarian causes of menstrual irregularity with associated elevated gonadotropins including LH and prolactin and arrested folliculogenesis typically observed in polycystic ovaries. Hyperprolactinaemia is usually considered a unique cause for oligo/amenorrhoea; however in the present case it was included due to the potential co-existence for elevated prolactin, LH and PCOS, [32, 35].

Although the exact cause of PCOS is not known, there are several factors that are associated with the condition. It is closely linked to high levels of hormones such as insulin and testosterone, but it is not clear if this is a cause or an effect of the condition. Additionally, it appears to run in some families, which suggests that there may be a genetic link in the pathogenesis of the condition.

Metformin has been tested in combination with cholesterol lowering medications. Other similar data show that combined treatment with metformin and atorvastatin compared to metformin alone produced similar but significant improvements in IR. [67] A moderate amount of daily exercise increases levels of IGF-1 binding protein and decreases levels of IGF-1 by 20%. Modest weight loss of 2-5% of total body weight can help restore ovulatory menstrual periods in obese patients with PCOS. A decrease of 500-1000 calories daily, along with 150 minutes of exercise per week, can cause ovulation.
There have been many studies on PCOS in the past several years; however, most are fairly small. Also, many studies examine medication effects on surrogate markers (e.g., androgen levels) rather than clinical outcomes (e.g., hirsutism). The catch is that it’s not safe in pregnancy, as it can cross the placenta and harm a fetus. For that reason, doctors prescribe spironolactone along with combination estrogen-progesterone birth control pills. I found out I had PCOS when i was 19 years old after I had lost a baby at 23 weeks along. I had gott...en my period 2 times a year so every 6 months.. I was told I wouldnt be able to have kids. Welm she had put me on medication for it and I have been on this medication for 2 years and I had lost 150 lbs and then in october of 2012 i found out I was pregnant again..
